SEHSCT - Primary Health care Feedback Questionnaire 2026 - Lakewood
Overview
Over the past year, the PHC team has made strong progress in addressing the key themes identified in last year’s survey, with a continued focus on supporting young people, strengthening collaboration with staff, and improving coordination with external health partners.
Key Achievements:
- Therapeutic Relationships: Increased presence within the home to build meaningful relationships and provide individualised health promotion to young people.
- Support for Floor Staff: Improved daily communication, alongside weekly medication audits and monthly care plan reviews to strengthen safe practice.
- GP and Pharmacy Liaison: Established a weekly GP MDT, ensuring all young people are seen within 7 days of admission. PHC now leads on prescription ordering, follow‑up, and collection when possible.
- Visibility and Responsiveness: Attendance at MDTs when invited, improved follow‑up of health concerns, and flexible support in response to the centre’s changing needs.

Priorities for the Year Ahead:
- Finalise and implement the new Medicines Management Policy, with staff training and competency assessments.
- Deliver in‑house BLS training, including naloxone administration.
- Develop a withdrawal‑management clinical pathway and strengthen support around substance misuse.
- Continue advocating for expanded PHC service hours in recognition of current Monday–Friday limitations.
- Review of Household Remedies.
