Overview

This end of project report is designed to provide Sport NI with valuable feedback on the delivery of your recent Creating Opportunities Award.

Please complete all questions within the report, we ask you to be honest and open in your feedback and report back accurate details as requested.

Completion and submission of this end of project report is a requirement within the Terms and Conditions of your award.

You must be an office bearer at your organisation and duly authorised to submit the information on behalf of the club. By submitting the report you are confirming that you are authorised to do so.

The report should take no longer than 15 minutes to complete.

The closing date for completed submissions is 30/4/25.
