Overview

The purpose of this form is to ascertain the status of community pharmacy during the COVID-19 pandemic.

As of 9th July 2020 business continuity sit-rep submissions are required to be submitted once a week, ideally on a Monday.

However if you are in a significant or escalating situation submit on any day, regardless of your last submission date.
