Closed 12 Jul 2020

Opened 30 Jun 2020

Overview

Since the start of the pandemic there have been a number of policy and practice measures implemented to prevent Covid-19 transmissions and mitigate the impact of the disease. We would like your help in identifying how effective those measures were and  what worked well. Your individual participation is very welcome. Your input will  help form recommendations that will be shared across health and social care services to support preparation for any future outbreak. Trust Care Home support teams are also invited to complete the survey. Thank you.

We require separate responses from care homes who provide nursing care and care homes who provide residential care. If your care home provides both, we will require you to complete 2 questionnaires. One for nursing and one for residential.

To do this please select 'Nursing'- complete the questionnaire and submit. Then click on the questionnaire link again and answer 'Residential' complete the questionnaire and submit.

It is imperative that no personal information is provided in response to this survey. This includes names, age, sex or any other information that may identify individuals.

Being transparent and providing accessible information to individuals is key to the Department of Health’s (DoH) commitment to building trust and confidence in our ability to process the information you share with us.

It is important therefore to note that your response, and all other responses to this consultation, may be disclosed on request in accordance with the Freedom of Information Act 2000 (FOIA) and the Environmental Information Regulations 2004 (EIR).
